tls: make deprecated tls.createSecurePair() use public API
Make the deprecated `tls.createSecurePair()` method use other public
APIs only (`TLSSocket` in particular).

Since `tls.createSecurePair()` has been runtime-deprecated only
since Node 8, it probably isn’t quite time to remove it yet,
but this patch removes almost all of the code complexity that
is retained by it.

The API, as it is documented, is retained. However, it is very likely
that some users have come to rely on parts of undocumented API
of the `SecurePair` class, especially since some of the existing
tests checked for those. Therefore, this should definitely be
considered a breaking change.

24dd92e77f net: use actual Timeout instance on Sockets
This makes `net.Sockets` use actual Timeout objects in a `[kTimeout]`
symbol property, rather than making the socket itself a timer and
appending properties to it directly.

This should make the code generally easier to understand, and might
also prevent some deopts from properties being changes on the socket
itself.

Also moves the Timeout constructor into an internal module.

446c1ecfda src, tools: add debug symbols for node internals
Before these changes, only V8 added debug symbols to Node's binary,
limiting the possibilities for debugger's developers to add some
features that rely on investigating Node's internal structures.

These changes are a first steps towards empowering debug tools to
navigate Node's internals strucutres. One example of what can be
achieved with this is shown at nodejs/llnode#122 (a command which prints
information about handles and requests on the queue for a core dump
file). Node debug symbols are prefixed with node_dbg_.

http2: major update to internals
This update does several significant things:

1. It eliminates the base Nghttp2* classes and folds those
   in to node::http2::Http2Session and node::http2::Http2Stream
2. It makes node::http2::Http2Stream a StreamBase instance and
   sends that out to JS-land to act as the [kHandle] for the
   JavaScript Http2Stream class.
3. It shifts some of the callbacks from C++ off of the JavaScript
   Http2Session class to the Http2Stream class.
4. It refactors the data provider structure for FD and Stream
   based sending to help encapsulate those functions easier
5. It streamlines some of the functions at the C++ layer to
   eliminate now unnecessary redirections
6. It cleans up node_http2.cc for better readability and
   maintainability
7. It refactors some of the debug output
8. Because Http2Stream instances are now StreamBases, they are
   now also trackable using async-hooks
9. The Stream::OnRead algorithm has been simplified with a
   couple bugs fixed.
10. I've eliminated node_http2_core.h and node_http2_core-inl.h
11. Detect invalid handshake a report protocol error to session
12. Refactor out of memory error, improve other errors
13. Add Http2Session.prototype.ping

build: Allow linking against an external copy of nghttp2.
The version of nghttp2 in deps/ does not build on CloudABI, even though
the official version does. Though this is an issue on its own that needs
to be resolved, it is currently a bit hard to work around this. There is
no switch to link against an external version of nghttp2, even though we
do provide this option for other libraries.

This change adds configure flags, similar to the ones we have for
OpenSSL, zlib, http_parser, libuv, etc. and makes the dependency on
deps/nghttp2 optional.

c661dad086 build: fix cctest target --with-dtrace

b021403b2a test: --enable-static linked executable
The motivation for this commit is to enable projects embedding Node.js
and building with --enable-static to be able to run the test suite and

Currently when building with --enable-static no node executable
will be created which means that the tests (apart from the cctest) and
linter cannot be run.

This is currently a work in progress and works on MacOS but I need to
run the CI, and manually on different environments to verify that it
works as expected.

f5809d8e35 build: fix cctest compilation
Currently the cctest target compiles sources files even though they are
compiled for the node target. This is my fault as when I worked on the
task of getting the cctest to use the object files from the node target
I missed a few sources that were being included from node.gypi. This
also effects the build time as these sources are compiled twice.

This commit moves the conditions in question into the node target in
node.gyp. With this commit there should be no object files in
out/Release/obj.target/cctest/src/ (the path will vary depending on the
operating system being used).

cad1d1ff45 src: add openssl-system-ca-path configure option
The motivation for this commit is that we need to specify system CA
certificates when building node. While we are aware of the environment
variable NODE_EXTRA_CA_CERTS this is not a great solution as we build
an RPM and we also don't want users to be able to unset them.

The suggestion is to add a configure time property like this:

--openssl-system-ca-path=OPENSSL_SYSTEM_CA_PATH
             Use the specified path to system CA (PEM format) in
             addition to the OpenSSL supplied CA store or compiled-
             in Mozilla CA copy.

Usage example:
$ ./configure --openssl-system-ca-path=/etc/pki/tls/certs/ca-bundle.crt

This would add the specified CA certificates in addition to the ones
already being used.

http2,async-wrap: introduce AliasedBuffer class
This change introduces an AliasedBuffer class and updates asytnc-wrap
and http2 to use this class.

A common technique to optimize performance is to create a native buffer
and then map that native buffer to user space via JS array. The runtime
can efficiently write to the native buffer without having to route
though JS, and the values being written are accessible from user space.

While efficient, this technique allows modifications to user
space memory w/out going through JS type system APIs, effectively
bypassing any monitoring the JS VM has in place to track program state
modifications. The result is that monitors have an incorrect view
of prorgram state.

The AliasedBuffer class provides a future placeholder where this
technique can be used, but writes can still be observed. To achieve
this, the node-chakra-core fork will add in appropriate tracking logic
in the AliasedBuffer's SetValue() method. Going forward, this class can
evolve to support more sophisticated mechanisms if necessary.

67269fd7f3 perf_hooks: implementation of the perf timing API
An initial implementation of the Performance Timing API for Node.js.
This is the same Performance Timing API implemented by modern browsers
with a number of Node.js specific properties. The User Timing mark()
and measure() APIs are implemented, garbage collection timing, and
node startup milestone timing.

```js
const { performance } = require('perf_hooks');
performance.mark('A');
setTimeout(() => {
  performance.mark('B');
  performance.measure('A to B', 'A', 'B');
  const entry = performance.getEntriesByName('A to B', 'measure')[0];
  console.log(entry.duration);
}, 10000);
```

The implementation is at the native layer and makes use of uv_hrtime().
This should enable *eventual* integration with things like Tracing
and Inspection.

The implementation is extensible and should allow us to add new
performance entry types as we go (e.g. for measuring i/o perf,
etc).

Documentation and a test are provided.

src: Node implementation of v8::Platform
Node.js currently uses the V8 implementation of the DefaultPlatform
which schedules VM tasks on a V8 managed thread pool. Since the Node.js
event loop is not aware of these tasks, the Node.js process may exit

asynchronous wasm compilation lands in V8.

This PR introduces a Node.js specific implementation of the v8::Platform

inspector: enable async stack traces
Implement a special async_hooks listener that forwards information
about async tasks to V8Inspector asyncTask* API, thus enabling
DevTools feature "async stack traces".

The feature is enabled only on 64bit platforms due to a technical
limitation of V8 Inspector: inspector uses a pointer as a task id,
while async_hooks use 64bit numbers as ids.

To avoid performance penalty of async_hooks when not debugging,
the new listener is enabled only when the process enters a debug mode:

 - When the process is started with `--inspect` or `--inspect-brk`,
   the listener is enabled immediately and async stack traces
   lead all the way to the first tick of the event loop.

 - When the debug mode is enabled via SIGUSR1 or `_debugProcess()`,
   the listener is enabled together with the debugger. As a result,
   only async operations started after the signal was received
   will be correctly observed and reported to V8 Inspector. For example,
   a `setInterval()` called in the first tick of the event will not be
   shown in the async stack trace when the callback is invoked. This
   behaviour is consistent with Chrome DevTools.

Last but not least, this commit fixes handling of InspectorAgent's
internal property `enabled_` to ensure it's set back to `false`
after the debugger is deactivated (typically via `process._debugEnd()`).

e71e71b513 http2: introducing HTTP/2
At long last: The initial *experimental* implementation of HTTP/2.

This is an accumulation of the work that has been done in the nodejs/http2
repository, squashed down to a couple of commits. The original commit
history has been preserved in the nodejs/http2 repository.

This PR introduces the nghttp2 C library as a new dependency. This library
provides the majority of the HTTP/2 protocol implementation, with the rest
of the code here providing the mapping of the library into a usable JS API.

Within src, a handful of new node_http2_*.c and node_http2_*.h files are
introduced. These provide the internal mechanisms that interface with nghttp
and define the `process.binding('http2')` interface.

The JS API is defined within `internal/http2/*.js`.

There are two APIs provided: Core and Compat.

The Core API is HTTP/2 specific and is designed to be as minimal and as
efficient as possible.

The Compat API is intended to be as close to the existing HTTP/1 API as
possible, with some exceptions.

Tests, documentation and initial benchmarks are included.

The `http2` module is gated by a new `--expose-http2` command line flag.
When used, `require('http2')` will be exposed to users. Note that there
is an existing `http2` module on npm that would be impacted by the introduction
of this module, which is the main reason for gating this behind a flag.

When using `require('http2')` the first time, a process warning will be
emitted indicating that an experimental feature is being used.

To run the benchmarks, the `h2load` tool (part of the nghttp project) is
required: `./node benchmarks/http2/simple.js benchmarker=h2load`. Only
two benchmarks are currently available.

Additional configuration options to enable verbose debugging are provided:

```
$ ./configure --debug-http2 --debug-nghttp2
$ NODE_DEBUG=http2 ./node
```

The `--debug-http2` configuration option enables verbose debug statements
from the `src/node_http2_*` files. The `--debug-nghttp2` enables the nghttp
library's own verbose debug output. The `NODE_DEBUG=http2` enables JS-level
debug output.

The following illustrates as simple HTTP/2 server and client interaction:

(The HTTP/2 client and server support both plain text and TLS connections)

```jt client = http2.connect('http://localhost:80');
const req = client.request({ ':path': '/some/path' });
req.on('data', (chunk) => { /* do something with the data */ });
req.on('end', () => {
  client.destroy();
});
// Plain text (non-TLS server)
const server = http2.createServer();
server.on('stream', (stream, requestHeaders) => {
  stream.respond({ ':status': 200 });
  stream.write('hello ');
  stream.end('world');
});
server.listen(80);
```

```js
const http2 = require('http2');
const client = http2.connect('http://localhost');
```

bb88caec06 build: fix ninja build failure
When working on commit 6a09a69ec9
("build: enable cctest to use generated objects") I did not take into
account building with ninja:

$ ./configure
$ tools/gyp_node.py -f ninja
$ ninja -C out/Release
$ ln -fs out/Release/node node

When ninja generated the ninja build files, src files that are
relative to the src directory will be named with a dot instead of a
path separator, for example:

out/Release/obj/src/node/node.o
would instead become:
out/Release/obj/src/node.node.o

This commit adds an additional variable for the type of object separator
used for this case.

Currently the check for if ninja is being used is a normal if statement
as are the following os checks (win and aix). But the win and aix ones
should only be evaluated if the build is not generated by ninja.
This commit turns this logic into an if ninja else statement.

test: add basic cctest for base64.h
This commit adds C++ tests for `base64_encode()` and `base64_decode()`
functions defined in `base64.h`. The functionality is already being
tested indirectly in JavaScript tests for Buffer, but it won't hurt to
test the low-level functions too, especially given that they aren't only
used in the internal Buffer implementation, Chrome inspector protocol
support relies upon them too.

n-api: add support for abi stable module API
Add support for abi stable module API (N-API) as "Experimental feature".
The goal of this API is to provide a stable Node API for native
module developers. N-API aims to provide ABI compatibility guarantees
across different Node versions and also across different
Node VMs - allowing N-API enabled native modules to just work
across different versions and flavors of Node.js without recompilation.

A more detailed introduction is provided in:
https://github.com/nodejs/node-eps/blob/master/005-ABI-Stable-Module-API.md
and https://github.com/nodejs/abi-stable-node/blob/doc/VM%20Summit.pdf.

The feature, during its experimental state, will be guarded by a runtime
flag "--napi-modules". Only when this flag is added to the command line
will N-API modules along with regular non N-API modules be supported.

The API is defined by the methods in "src/node_api.h" and
"src/node_api_types.h". This is the best
starting point to review the API surface. More documentation will follow.

In addition to the implementation of the API using V8, which is included
in this PR, the API has also been validated against chakracore and that
port is available in
https://github.com/nodejs/abi-stable-node/tree/api-prototype-chakracore-8.x.

The current plan is to provide N-API support in versions 8.X and 6.X
directly. For older versions, such as 4.X or pre N-API versions of 6.X,
we plan to create an external npm module to provide a migration path
that will allow modules targeting older Node.js versions to use the API,
albeit without getting the advantage of not having to recompile.

In addition, we also plan an external npm package with C++ sugar to
simplify the use of the API. The sugar will be in-line only and will
only use the exported N-API methods but is not part of the N-API
itself. The current version is in:
https://github.com/nodejs/node-api.

This PR is a result of work in the abi-stable-node repo:
https://github.com/nodejs/abi-stable-node/tree/doc,
with this PR being the cumulative work on the api-prototype-8.x
branch with the following contributors in alphabetical order:

v8: expose new V8 serialization API
Expose the new serialization API that was added in V8 5.5 to userland.
The JS API is virtually a direct copy of what V8 provides on the

This is useful Node as a possible replacement for some internals

general userland code as well.

6a09a69ec9 build: enable cctest to use generated objects
This commit tries to make it simpler to add unit tests (cctest) for
code that needs to test node core funtionality but that might not be
appropriate as an addon or a JavaScript test. An example of this could
be adding functionality targeted for situations when Node itself is
embedded.

Currently it was not as easy, or efficient, as one would have hoped to
add such tests. The object output directories vary for different
operating systems which we need to link to so that we don't have an
additional compilation step.

70afe9d3ac build: fix building with ninja on linux
On Linux, `ninja` appears to place `libv8_base.a` inside `OBJ_DIR`, as opposed
to `ninja` on OS X which places it outside of that directory. Furthermore, the
expected `OBJ_DIR` value (`obj.target/`) is actually just `obj/` for `ninja`.
This patch solves both of these issues by setting `OBJ_DIR` and `V8_BASE` to the
correct values for `ninja` on Linux specifically.

ba4847e879 src: Node Tracing Controller
This commit adds support for trace-event tracing to Node.js. It provides
a mechanism to centralize tracing information generated by V8, Node
core, and userspace code. It includes:

 - A trace writer responsible for serializing traces and cycling the
   output files so that no individual file becomes to large.
 - A buffer for aggregating traces to allow for batched flushes.
 - An agent which initializes the tracing controller and ensures that
   trace serialization is done on a separate thread.
 - A set of macros for generating trace events.
 - Tests and documentation.

didn't support them was released in 2007 so I think it's safe to assume
we can drop support for that now - except on SmartOS, where the tooling
still has a distinctive vintage feel to it.

Thin archives save space - it shrinks the size of PRODUCT_DIR by 30% -
and speed up the final linking step because it doesn't have to assemble
50 MB of static archives (twice! - first to create the archive, then to
copy it to PRODUCT_DIR). The archives are just 3.5 MB now and no longer
copied around.

c6656db352 process: add 'warning' event and process.emitWarning()
In several places throughout the code we write directly to stderr
to report warnings (deprecation, possible eventemitter memory leak).
The current design of simply dumping the text to stderr is less
than ideal. This PR introduces a new "process warnings" mechanism
that emits 'warning' events on the global process object. These are
invoked with a `warning` argument whose value is an Error object.

By default, these warnings will be printed to stderr. This can be
suppressed using the `--no-warnings` and `--no-deprecation` command
line flags. For warnings, the 'warning' event will still be emitted
by the process, allowing applications to handle the warnings in custom
ways. The existing `--no-deprecation` flag will continue to supress
all deprecation output generated by the core lib.

The `--trace-warnings` command line flag will tell Node.js to print
the full stack trace of warnings as part of the default handling.

The existing `--no-deprecation`, `--throw-deprecation` and
`--trace-deprecation` flags continue to work as they currently do,
but the exact output of the warning message is modified to occur
on process.nextTick().

The stack trace for the warnings and deprecations preserve and point
to the correct call site.

A new `process.emitWarning()` API is provided to permit userland
to emit warnings and deprecations using the same consistent
mechanism.

Test cases and documentation are included.

d387591bbb src: add --security-revert command line flag
The `--security-revert={cvenum}` command line flag is a special purpose
flag to be used only in stable or LTS branches when a breaking change
is required to address a security vulnerability. Whenever a vulnerability
requires a breaking change, and a CVE has been assigned, the flag can
be used to force Node to revert to the insecure behavior that was
implemented before the fix was applied.

Note that this flag is intended to be used only as a last resort in the
case a security update breaks existing code. When used, a security
warning will be printed to stderr when Node launches.

The `--security-revert={cvenum}` flag takes a single CVE number as an
argument. Multiple instances of the `--security-revert={cvenum}` flag
can be used on the command line to revert multiple changes.

Whenever a new `--security-revert={cvenum}` is enabled, it should be
documented in the release notes and in the API docs.

Master and the first release of a new major (e.g. v6.0) should not have
any reverts available.

Every time a new `--security-revert={cvenum}` is added, there should be
a semver-minor bump in the stable and LTS branch.

15bcbf853e build: Updates for AIX npm support - part 1
This PR is the first step enabling support for native modules
for AIX. The main issue is that unlike linux where all
symbols within the Node executable are available to the shared
library for a native module (npm), on AIX the symbols must
be explicitly exported. In addition, when the shared library is
built it must be linked using a list of the available symbols.

This patch covers the changes need to:

 1) Export the symbols when building the node executable
 2) Generate the file listing the symbols that can be used when
    building the shared library.

For AIX, it breaks the build process into 2 steps. The first builds
a static library and then generates a node.exp file which contains
the symbols from that library. The second builds the node executable
and uses the node.exp file to specify which symbols should be
exported. In addition, it save the node.exp file so that it can
later be used in the creation of the shared library when building
a native module.

The following additional steps will be required in dependent projects
to fully enable AIX for native modules and are being worked
separately:

 - Updates to node-gyp to use node.exp when creating the
   shared library for a native module
 - Fixes to gyp related to copying files as covered in
      https://codereview.chromium.org/1368133002/patch/1/10001
 - Pulling in updated gyp versions to Node and node-gyp
 - Pulling latest libuv

These changes were done to minimize the change to other platforms
by working within the existing structure to add the 2 step process
for AIX without changing the process for other platforms.

b9686233fc stream_base: introduce StreamBase
StreamBase is an improved way to write C++ streams. The class itself is
for separting `StreamWrap` (with the methods like `.writeAsciiString`,
`.writeBuffer`, `.writev`, etc) from the `HandleWrap` class, making
possible to write abstract C++ streams that are not bound to any uv
socket.

The following methods are important part of the abstraction (which
mimics libuv's stream API):

* Events:
  * `OnAlloc(size_t size, uv_buf_t*)`
  * `OnRead(ssize_t nread, const uv_buf_t*, uv_handle_type pending)`
  * `OnAfterWrite(WriteWrap*)`
* Wrappers:
  * `DoShutdown(ShutdownWrap*)`
  * `DoTryWrite(uv_buf_t** bufs, size_t* count)`
  * `DoWrite(WriteWrap*, uv_buf_t*, size_t count, uv_stream_t* handle)`
  * `Error()`
  * `ClearError()`

The implementation should provide all of these methods, thus providing
the access to the underlying resource (be it uv handle, TLS socket, or
anything else).

A C++ stream may consume the input of another stream by replacing the
event callbacks and proxying the writes. This kind of API is actually
used now for the TLSWrap implementation, making it possible to wrap TLS
stream into another TLS stream. Thus legacy API calls are no longer
required in `_tls_wrap.js`.

4f68369643 build: disable v8 snapshots
Snapshots speed up start-up by a few milliseconds but are potentially
dangerous because of the fixed hash seed that is used for strings and
dictionaries, making collision denial-of-service attacks possible.

Release builds on iojs.org have snapshots disabled but source builds
did not, until now.

The risk for individual source builds is low; the binary gets a random
32 bits hash seed that should be hard to guess by an external attacker.

It's when binaries are distributed by, for example, a distro vendor
that the fixed hash seed becomes a vulnerability, because then it's
possible to target a large group of people at once.

People that really need the faster start-up time can use the new
--with-snapshot configure flag.

ac2857b12c build, i18n: improve Intl build, add "--with-intl"
The two main goals of this change are:
 - To make it easier to build the Intl option using ICU (particularly,
   using a newer ICU than v8/Chromium's version)
 - To enable a much smaller ICU build with only English support The goal
   here is to get node.js binaries built this way by default so that the
   Intl API can be used. Additional data can be added at execution time
   (see Readme and wiki)

More details are at https://github.com/joyent/node/pull/7719

In particular, this change adds the "--with-intl=" configure option to
provide more ways of building "Intl":
 - "full-icu" picks up an ICU from deps/icu
 - "small-icu" is similar, but builds only English
 - "system-icu" uses pkg-config to find an installed ICU
 - "none" does nothing (no Intl)

For Windows builds, the "full-icu" or "small-icu" options are added to
vcbuild.bat.

Note that the existing "--with-icu-path" option is not removed from
configure, but may not be used alongside the new option.

Wiki changes have already been made on
 https://github.com/joyent/node/wiki/Installation
and a new page created at
 https://github.com/joyent/node/wiki/Intl
(marked as provisional until this change lands.)

Summary of changes:

* README.md : doc updates

* .gitignore : added "deps/icu" as this is the location where ICU is
  unpacked to.

* Makefile : added the tools/icu/* files to cpplint, but excluded a
  problematic file.

* configure : added the "--with-intl" option mentioned above.
  Calculate at config time the list of ICU source files to use and data
  packaging options.

* node.gyp : add the new files src/node_i18n.cc/.h as well as ICU
  linkage.

* src/node.cc : add call into
  node::i18n::InitializeICUDirectory(icu_data_dir) as well as new
  --icu-data-dir option and NODE_ICU_DATA env variable to configure ICU
  data loading. This loading is only relevant in the "small"
  configuration.

* src/node_i18n.cc : new source file for the above Initialize..
  function, to setup ICU as needed.

* tools/icu : new directory with some tools needed for this build.

* tools/icu/icu-generic.gyp : new .gyp file that builds ICU in some new
  ways, both on unix/mac and windows.

* tools/icu/icu-system.gyp : new .gyp file to build node against a
  pkg-config detected ICU.

* tools/icu/icu_small.json : new config file for the "English-only" small
  build.

* tools/icu/icutrim.py : new tool for trimming down ICU data. Reads the
  above .json file.

* tools/icu/iculslocs.cc : new tool for repairing ICU data manifests
  after trim operation.

* tools/icu/no-op.cc : dummy file to force .gyp into using a C++ linker.

* vcbuild.bat : added small-icu and full-icu options, to call into
  configure.

* Fixed toolset dependencies, see
  https://github.com/joyent/node/pull/7719#issuecomment-54641687

Note that because of a bug in gyp {CC,CXX}_host must also be set.
Otherwise gcc/g++ will be used by default for part of the build.

c4def505cb build: use zero overhead systemtap probes
Profiling suggested that on Linux sometimes over 10% of CPU time was
being spent inside the systemtap probe entry points in the binding
layer, even when the process was not actively being traced with the
`stap` tool.

That's why this commit makes it possible to use the *_ENABLED() macros
and bail out early when we're not being traced, reducing the overhead
of unused probes to (almost) zero.

Said macros were already being generated by `dtrace -h` but were not
usable because they rely on external definitions. To remedy that, we
now generate the accompanying object files with `dtrace -G`.

This commit includes a change to libuv that has been landed upstream in
commit joyent/libuv@3c172ea.

756b622295 src: add multi-context support
This commit makes it possible to use multiple V8 execution contexts
within a single event loop. Put another way, handle and request wrap
objects now "remember" the context they belong to and switch back to
that context when the time comes to call into JS land.

This could have been done in a quick and hacky way by calling
v8::Object::GetCreationContext() on the wrap object right before
making a callback but that leaves a fairly wide margin for bugs.

Instead, we make the context explicit through a new Environment class
that encapsulates everything (or almost everything) that belongs to
the context. Variables that used to be a static or a global are now
members of the aforementioned class. An additional benefit is that
this approach should make it relatively straightforward to add full
isolate support in due course.

There is no JavaScript API yet but that will be added in the near
future.

7afdba6e0b vm, core, module: re-do vm to fix known issues
As documented in #3042 and in [1], the existing vm implementation has
many problems. All of these are solved by @brianmcd's [contextify][2]
package. This commit uses contextify as a conceptual base and its code
core to overhaul the vm module and fix its many edge cases and caveats.

Functionally, this fixes #3042. In particular:

- A context is now indistinguishable from the object it is based on
  (the "sandbox"). A context is simply a sandbox that has been marked
  by the vm module, via `vm.createContext`, with special internal
  information that allows scripts to be run inside of it.
- Consequently, items added to the context from anywhere are
  immediately visible to all code that can access that context, both
  inside and outside the virtual machine.

This commit also smooths over the API very slightly:

- Parameter defaults are now uniformly triggered via `undefined`, per
  ES6 semantics and previous discussion at [3].
- Several undocumented and problematic features have been removed, e.g.
  the conflation of `vm.Script` with `vm` itself, and the fact that
  `Script` instances also had all static `vm` methods. The API is now
  exactly as documented (although arguably the existence of the
  `vm.Script` export is not yet documented, just the `Script` class
  itself).

In terms of implementation, this replaces node_script.cc with
node_contextify.cc, which is derived originally from [4] (see [5]) but
has since undergone extensive modifications and iterations to expose
the most useful C++ API and use the coding conventions and utilities of
Node core.

The bindings exposed by `process.binding('contextify')`
(node_contextify.cc) replace those formerly exposed by
`process.binding('evals')` (node_script.cc). They are:

- ContextifyScript(code, [filename]), with methods:
  - runInThisContext()
  - runInContext(sandbox, [timeout])
- makeContext(sandbox)

From this, the vm.js file builds the entire documented vm module API.

node.js and module.js were modified to use this new native binding, or
the vm module itself where possible. This introduces an extra line or
two into the stack traces of module compilation (and thus into most
stack traces), explaining the changed tests.

The tests were also updated slightly, with all vm-related simple tests
consolidated as test/simple/test-vm-* (some of them were formerly
test/simple/test-script-*). At the same time they switched from
`common.debug` to `console.error` and were updated to use
`assert.throws` instead of rolling their own error-testing methods.

New tests were also added, of course, demonstrating the new

34b0a36120 src: don't use NewExternal() with unaligned strings
V8 3.20.9 enforces that external pointers are aligned on a two-byte
boundary.

We cannot portably guarantee that for the source code strings that
tools/js2c.py generates so simply stop using String::NewExternal()
altogether (and by extension String::ExternalAsciiStringResource).

Fixes the following run-time assert:

  FATAL ERROR: v8::String::NewExternal() Pointer is not aligned

4b279f0092 src: rename macros.py to notrace_macros.py
This incarnation of macros.py is only used to disable the (d)trace
macros. Rename it so it better reflects its purpose. A new macros.py
will be added in a follow-up commit.

d817843d2e smalloc: create separate module
It will be confusing if later on we add Buffer#dispose(), and smalloc is
its own cpp api anyways. So instead create a new require('smalloc') to
expose the previous Buffer.alloc/dispose methods, and expose copyOnto
and kMaxLength as well.

Other changes:
* Added documentation and additional tests.
* smalloc::CopyOnto has changed from using assert() to throwing errors
  on bad argument values because it is not exposed to the user.
* Minor style fixes.

110a9cd8db lib, src: upgrade after v8 api change
This is a big commit that touches just about every file in the src/
directory. The V8 API has changed in significant ways. The most
important changes are:

* Binding functions take a const v8::FunctionCallbackInfo<T>& argument
  rather than a const v8::Arguments& argument.

* Binding functions return void rather than v8::Handle<v8::Value>. The
  return value is returned with the args.GetReturnValue().Set() family
  of functions.

* v8::Persistent<T> no longer derives from v8::Handle<T> and no longer
  allows you to directly dereference the object that the persistent
  handle points to. This means that the common pattern of caching
  oft-used JS values in a persistent handle no longer quite works,
  you first need to reconstruct a v8::Local<T> from the persistent
  handle with the Local<T>::New(isolate, persistent) factory method.

A handful of (internal) convenience classes and functions have been
added to make dealing with the new API a little easier.

The most visible one is node::Cached<T>, which wraps a v8::Persistent<T>
with some template sugar. It can hold arbitrary types but so far it's
exclusively used for v8::Strings (which was by far the most commonly
cached handle type.)

ec90e6e80a slab_allocator: remove SlabAllocator
Now that Buffer instantiation has improved, the SlabAllocator is an
unnecessary layer of complexity preventing further performance
optimizations.

Currently there is a small performance loss with very small stream
requests, but this will soon be addressed.

8f3f9f7830 smalloc: initial implementation
smalloc is a simple utility for quickly allocating external memory onto
js objects. This will be used to centralize how memory is managed in
node, and will become the backer for Buffers. So in the future crypto's
SlabBuffer, stream's SlabAllocator will be removed.

Note on the js API: because no arguments are optional the order of
arguments have been placed to match their cc counterparts as closely as
possible.
