Sam Ruby 46de34b8c2 tools: build API TOC using raw headers
Markdown interprets the syntax for optional arguments as a form
of a link, so instead of trying to build up the contents using
the node value, use grab the raw original markup instead.

Fixes regression noted in
https://github.com/nodejs/node/pull/21490#issuecomment-406785023

1:1 relationship from lib/<module>.js to doc/api/<module>.md.

Each type of heading has a description block.

  * `field` {string} `anArg` can have this field.

`SomeClass` must be registered in `tools/doc/type-parser.js`
to be properly parsed in `{type}` fields.

This event is emitted on instances of `SomeClass`, not on the module itself.

  • Events have (list of listener arguments, description).

  • Methods have (list of arguments, returned value if defined, description).
  • Modules have (description, Properties, Functions, Classes, Examples).
