As the CTC grows and has representation from more time zones, we need to
embrace asynchronous decision making and rely less on the actual
meeting. This change is a proposal for that which, ironically, probably
has to be approved at a meeting.

CTC quorum rules were not in writing. There was an informal
understanding between CTC members. Document the rules to avoid
differences in interpretation.

This commit replaces instances of io.js with Node.js, based on the
recent convergence. There are some remaining instances of io.js,
related to build and the installer.
