http2,doc: minor fixes
Some small fixes on HTTP/2 and its documentation:

 - Add a note that, on server streams, it's not necessary
   to start data flow.

 - Set EOF flag if we have marked all data for sending:
   there's no need to wait until the queue is
   actually empty (and send a separate, empty DATA).

   (Note that, even with this change, a separate DATA
   frame will always be sent, because the streams
   layer waits until data has been flushed before
   dispatching EOF)

doc: fix LTS replaceme tags
When cherry-picking release commits for LTS releases into master,
the `REPLACEME` metadata can be taken over as well, to give users
a more accurate view of what is being released on which release line.

This addresses this problem for all previous LTS releases for which
this has not been done.

Both http and https modules have server.timeout property
in public API. This commit adds documentation section and test
for server.timeout in http2 module, so it becomes consistent
with http and https.

b6cd2155c3 doc: remove em dashes
Our documentation uses em dashes inconsistently. They are treated
inconsistently typographically too. (For example, they are sometimes
surrounded by spaces and sometimes not.) They are also often confused
with ordinary hyphens such as in the CHANGELOG, where they are
inadvertently mixed together in a single list. The difference is
not obvious in the raw markdown but is very noticeable when rendered,
appearing to be a typographical error (which it in fact is).

The em dash is never needed. There are always alternatives. Remove em
dashes entirely.

ac887f849a doc: standardize on "host name" in http2.md
Our docs have a mix of "hostname" and "host name" in prose.

Let's follow the usage of Unix man pages, RFCs, and most
professionally-edited sources, and use "host name" in prose and
"hostname" to refer to the command and in code.

10040500da doc: remove dashes
The use of dashes -- in general, but especially in our docs -- can be
problematic. It is used inconsistently and there is always another form
of punctuation that is as good or better for the situation. In an effort
to reduce the number of variations we use to display the same types of
information, remove the various uses of dashes from the documentation.

6f613d8abb http,stream: add writableEnded
This is work towards resolving the response.finished confusion and
future deprecation.

Note that implementation-wise, streams have both an ending and ended
state. However, in this case (in order to avoid confusion in user space)
writableEnded is equal to writable.ending. The ending vs ended situation
is internal state required for internal stream logic.

doc: clarify http2 server.close() behavior
This commit is an attempt to clarify the behavior of HTTP2's
server.close() method. Specifically, this makes it more clear
that the server stops allowing new sessions although the
callback may not be invoked for some time due to previously
existing sessions.
